Résumé
La dépendance et les autres troubles mentaux interagissent de plusieurs façons. L’abus de substances psychoactives a tendance à exacerber les symptômes psychiatriques et à favoriser la chronicité, avec des périodes sans troubles, plus rares et plus courtes. L’abus de substances s’oppose aussi à l’efficacité des traitements psychiatriques. La co-existence d’un trouble psychiatrique aggrave l’évolution de la dépendance. Les sujets dépendants et psychiatriques ont tendance à avoir un mode de vie désordonné et sont enclins à des conduites à risque.
Enfin, le risque de rechutes est souvent si élevé qu’il décourage toute intervention thérapeutique. Dans cet article nous mettons l’accent sur certains aspects particulièrement importants du traitement de maintenance et proposons des recommandations pour la pratique clinique. Les auteurs ont participé à un travail collectif pour analyser l’ensemble des comorbidités. Cet article s’appuie sur les données de la littérature et sur l’expérience clinique des auteurs dans leurs centres de soins respectifs.
Nous suggérons que les patients dépendants à double diagnostic (DD) soient traités dans un premier temps pour leur dépendance en utilisant des posologies appropriées de méthadone, posologies généralement supérieures à celles utilisées chez les patients dépendants sans autre complication. La stabilisation doit être considérée comme l’objectif intermédiaire. Certains patients à double diagnostic peuvent bénéficier d’un traitement centré sur leur dépendance tout en prenant en compte leurs troubles psychiatriques. Indépendemment de leurs propriétés anticompulsives, les agonistes des opiacés doivent être ré-évalués et considérés comme des psychotropes pour le traitement des troubles mentaux, en particulier les troubles de l’humeur et de l’anxiété ainsi que les syndromes psychotiques. Enfin, tout comme les sujets dépendants sans trouble associé, les sujets dépendants à double diagnostic devraient bénéficier d’une prise en charge globale, dans la mesure où ces traitements sont basés sur l’utilisation de posologies adaptées pendant un temps de traitement suffisamment long.
